Replacing a lost or stolen passport.
If your existing U.S. Passport has been lost or stolen, then you are at the right place to have it replaced and returned specially if you're in a rush. The following is a list of things that you need to give us in order for us to rush a relplacement to you. In addition, you will have to sign certain forms in front of an Acceptance Agent, so please take the documents mentioned below with you when going to the Acceptance Agency.

1) Form DSP-11 For more information on downloading this form, please click on "get forms", on the menu to your left. *Please, do not sign these forms before going to the Acceptance Agency. You must sign them in front of the Acceptance Agent otherwise the forms will be deemed invalid.*

2) Form DSP-64 This form is a statement regarding your lost or stolen passport.

3) Two identical passport photos. For more information on requirements, click on "photos" on the menu to your left.

4) Proof of citizenship, in the form of a certified birth certificate, previous passport or certificate of naturalization. If your name has been legally changed, submit official documentation as proof.

5) A check made out to Speedy Passports. A small 'Rush' fee will be added.
a) For the amount of $157.00 if you would like to have your service expedited. (1-5 days)
b)For the amount of $127.00 if you're in a rush you can receive your passport much quicker than normal. (more than 5 days)

6) Proof of identity. Make sure you take a valid form of identification such as a driver's license when you go to the Acceptance Agency to sign the necessary forms.

7) Proof of departure, such as your travel itinerary or copy of airplane tickets, necessary for us in order to expedite your passport application process.

8) A filled out copy of our agreement.

When you are in front of the Acceptance Agent, ask them to seal the items listed above in an official envelope, and let them know that you plan on applying for your passport through a passport expediting service. Without opening the envelope, send it to us. If you're in a rush it's important to follow the above steps properly.
